Waiver & Liability Policy
By registering for this UCR programs, I am admitting I am fully aware of the risks and hazards connected to indoor/outdoor physical activity, vocational training and/or manual labor. I am also aware that some of the programs will include animal therapy/interactions and/or travel in the community.
By attending in person programs, you are acknowledging you will be sharing spaces with other UCR attendees.
I hereby release, waive, discharge and covenant not to sue Upcounty Community Resources, Inc, their program partner sites, their officers, employees, volunteers or assistants from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any injury that may be sustained by me or my charge while enrolled in UCR programming.

Facility Security Video Surveillance Disclosure & Consent:
Because our organization leases properties that operate across multiple floors without building-wide security features provided by the landlord, and without an intermediary security vestibule or physical lockdown barrier, the facility faces an elevated security vulnerability regarding unauthorized entry or street-level intrusion. Video surveillance is deployed strictly as an environmental security control to deter external threats, protect vulnerable adult DDA participants and minors, and provide verifiable forensic evidence in the event of an incident
This system is completely passive, is never live-streamed, and has all audio/microphone features permanently disabled in compliance with Maryland law. Footage is stored securely on password-protected cloud servers, is strictly closed to the public and general staff, and is only reviewed by authorized administrators following a formally documented safety incident, injury, or unauthorized building entry. All footage automatically and permanently deletes every 30 days.
By completing this enrollment registration, you acknowledge this security measure and grant express consent for the participant to be recorded passively under these strict privacy guardrails.
